Good Faith Estimate Notice

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.

What is your rescheduling and

cancellation policy?

I appreciate as much notice as possible when it is necessary to cancel or reschedule an appointment.   I have a strict 24 hour cancellation policy.   My fee for canceled appointments with less than 24 hours notice is $50.00, when appointment is rescheduled at time of cancellation.

What is a Marriage and Family Therapist?

Marriage and Family Therapists (MFTs) are relationship specialists who treat persons involved in interpersonal relationships. They are trained to assess, diagnose and treat individuals, couples, families and groups, treat mental illness and psychological distress within the context of the marriage and family systems to achieve a more adequate, satisfying and productive marriage, improve family interactions and social adjustments. My practice also includes premarital counseling, divorce or separation counseling and other relationship counseling. Marriage and Family Therapists are psychotherapists and healing arts practitioners licensed by the State of California. Requirements for licensure include a related master's or doctoral degree, passage of comprehensive written examinations and at least 3,000 hours of supervised clinical experience working with patients in a mental health capacity.
